We are required to verify your identity under the Anti-Money Laundering and Counter-Terrorism Financing Act. Provide details for each applicant and attach certified copies.

Providing a Tax File Number is not compulsory. If not quoted, withholding tax may be deducted from certain income at the highest marginal rate. Certified copies of identification must accompany this form.

This information helps us assess whether an investment is appropriate for you and satisfies our regulatory obligations. All figures are treated in strict confidence.

1. I/we declare that the information provided in this application is true, accurate and complete, and I/we will promptly notify Matic Capital of any material change.
2. I/we acknowledge that I/we have received and read the Matic Capital prospectus and the Legal & Disclosures information, and understand that investing involves risk, including possible loss of capital, and that past performance is not a reliable indicator of future performance.
3. I/we understand that Matic Capital provides general information and, where engaged, personal advice under AFSL 277313, and that investments are made in United States–listed assets as disclosed.
4. I/we consent to Matic Capital collecting, using and disclosing my/our personal information in accordance with its Privacy Policy and its obligations under AML/CTF law.
